Please print out the form, fill it in, sign it and send it back to us by scanning it and emailing it, or by post. The filling in can be done by typing on the document on line, if you prefer, before you print it out and sign it. If the counselling is ‘couples counselling’ for yourself and your partner together (rather than individual counselling for yourself only) then your partner will need to complete the relevant parts of the form. As soon as we receive the form we will contact you with a date for a first appointment. We aim for this appointment to be within 10 days of receipt of your request. This will provide an opportunity for you to discuss your situation with a counsellor and for an assessment to be made as to the most suitable form of counselling help.
